Complete this form to remove a person in control of your transport service licence. The Land Transport Rule: Operator Licensing 2017 section 2.4 requires you to let us know within 14 days of the change.
To add a new person in control, you'll need to complete a different form.
We'll only accept TL04 applications from a current person in control of the transport service licence.
If that isn't you, please ask an appropriate person to complete this form instead.
The information you provide with this application will be used to update your transport service licence details and to maintain the register of transport service licences. Collection of this information is required by part 4A of the Land Transport Act 1998 and section 2 of the Land Transport Rule: Operator Licensing 2017. Collection, dissemination and release of this information is authorised by the Land Transport Act and part 7 of the Privacy Act 2020.
Waka Kotahi NZ Transport Agency will hold, store, use or disclose personal information collected through this application in accordance with the Privacy Act 1993. Waka Kotahi may also use some of these particulars to provide you with information relating to transport service licensing or to contact you for feedback on our services.
