Complete this form to remove a person in control of your transport service licence. The Land Transport Rule: Operator Licensing 2017 section 2.4 requires you to let us know within 14 days of the change.

To add a new person in control, you'll need to complete a different form.

At least one person in control of a goods, large passenger or vehicle recovery service must hold a Certificate of knowledge of law and practice. If you've removed the person in control that used to hold the certificate of knowledge, you'll need to tell us who the new person is that holds a certificate of knowledge.

Collection of this information is required by part 4A of the Land Transport Act 1998 and section 2 of the Land Transport Rule: Operator Licensing 2017. Collection, dissemination and release of this information is authorised by the Land Transport Act and part 7 of the Privacy Act 2020.

Waka Kotahi may also use some of these particulars to provide you with information relating to transport service licensing or to contact you for feedback on our services.
