Use this form to tell us if you've received a safety camera infringement or traffic offence notice but aren't responsible for the infringement or offence. For example:
- someone else was driving
- you sold the vehicle, or
- the vehicle or plates were stolen.
You must tell us as soon as possible. We can't transfer liability after the final due date on your reminder notice.
If it's a traffic offence notice, you must tell us within 10 working days.
You can't transfer liability if you've already paid the fine. If you want to transfer liability to someone else, don't pay the fine (even if the other driver is a family member). Wait for the infringement to be transferred before making the payment.
Who can complete this form
You can only transfer a notice if you're:
- the person named on the notice, or
- a person they've given specific authority to act on their behalf, or
- an authorised representative of a company or organisation.
What you need to supply
You must provide a photo or scanned copy of a correctly completed statutory declaration. We won't accept declarations if they're incomplete.

If you have authority to act on someone's behalf and you're completing this form for them, you must include your details on the statutory declaration.
Your statutory declaration must be witnessed.

After you've submitted this form
We'll process your request and tell you if we've accepted or declined it, and why.
If the statutory declaration is:
- accepted, we'll transfer the notice to the person you named and send them a new notice.
- not accepted, you must either pay the infringement fee or contact NZTA before the final due date and tell us you want to take other action.
