Ask for a copy of your safety camera notice

Use this form to request a replacement copy of a safety camera notice issued by NZ Transport Agency Waka Kotahi (NZTA).

Your infringement notices remains payable to NZTA by the original due date. Requesting a copy of the notice doesn’t extend the due date. 

Who can complete this form

You can only request a replacement copy if you're:

• the person named on the notice, or
• a person they've given specific authority to act on their behalf, or
• an authorised representative of a company or organisation.

If you're acting on behalf

If the notice was issued to a company or organisation, you must be entitled to complete this form on their behalf. Check with a director or manager if you're not sure.

If the notice is for an individual, make sure they've told us you can act on their behalf. We can't send you a copy of the notice otherwise.
