Notification to add a person in control of transport service licence (TL04)

Complete this form to add a new person in control to your transport service licence. The Land Transport Rule: Operator Licensing 2017 section 2.4 requires you to tell us within 14 days of the change.

To remove a person in control, you'll need to complete a different form.

Before you fill in the fields below, you'll need to pay a vetting fee of $9.70 for each new person in control being added to your licence.

Supporting documents

For each new person in control, you must submit:

  • a completed Personal details in support of a transport service licence (TL02) form
  • a certified copy of their passport or New Zealand driver licence.

A certified copy must be stamped or endorsed by a person who confirms it's a true copy of the original document. This can be a:

  • Justice of the Peace
  • Solicitor of the High Court 
  • Notary Public 
  • Deputy Registrar at a court (not all courts will be able to certify copies).

Collection of this information is required by part 4A of the Land Transport Act 1998 and section 2 of the Land Transport Rule: Operator Licensing 2017. Collection, dissemination and release of this information is authorised by the Land Transport Act and part 7 of the Privacy Act 2020.
