Below are the current 50MAX proforma designs. Download the PDFs to fill in and submit as part of your 50MAX application (Note: you can save the PDFs to your computer to upload with your application).
Note: Truck drive axles and trailer axles must be twin-tyred axles.
The use of these vehicles are prohibited in a proforma combination for 50MAX due to the pavement damage caused when conducting a turn. The exceptions are the two special proformas that require the truck to have a rear steering axle that locks above 60km/h. These vehicles however, can apply for a HPMV Overlength and Higher Mass permit to operate on if operating above general access limits.
Note the new PDF 50MAX HPMV proforma forms provide an option for a heavy vehicle specialist certifier to certify the dimension limits – this certification is only required for new non-registered vehicles to enable entry certification and registration as an option instead of submitting a 50MAX HPMV length permit to the testing station. The permit can then be applied for after registration.
